By purchasing this ticket, I hereby acknowledge that I am voluntarily participating in the fundraising event held by First Base Fitness and physiotherapy on May 11, 2024. I understand that the event will involve activities such as running on a treadmill, using a ski erg, bike, rower, and body weight work. I acknowledge and agree that there are inherent risks associated with physical activities, and I voluntarily assume all risks related to my participation in this event.
 
I hereby release, waive, discharge, and covenant not to sue First Base Fitness, its owners, employees, and affiliates from any and all liability, claims, demands, actions, or damages of any kind that may arise from my participation in this event.
 
I understand the nature of the activities involved and certify that I am physically fit, have sufficiently trained for participation, and have no medical conditions that would prevent me from safely engaging in the activities.
 
In the event of an injury or medical emergency, I authorize event staff to seek medical treatment on my behalf. I have read this waiver of liability and fully understand its contents. I agree to abide by all rules and instructions provided by event staff for the safety and well-being of all participants.
